Proof of Claim Form - A Message from FIA Steering Committee

Last evening, members of the FIA Steering Committee met on a conference call and
discussed in great lengths the issue of the Proof of Claim form that all
families will need to complete and return to BDO prior to July 30, 2009. With
the complexity of this form, we have been hesitant to state an opinion on how to
fill out without contacting the office of Susan Taves as the form can be
interpreted in many ways. For the time being, we as the Trustee Liaison
Committee are asking you to follow these steps:

• For those families that have not completed the form as of yet, please hold off
until we provide further information, which in honesty should be later today.

• We fully anticipate that our meetings and conversations will Susan Taves will
come to close later today and we will be in a position to formally post some
guidance for your assistance. This post as it stands will either be directly
through BDC or will be subsequent email from the FIA Steering Committee.

• We do note that for those families that have already completed the forms,
please do not panic if you reference information later today that is different
than the way you completed the form, as the form can be amended anytime.

We do fully understand that you are awaiting our information on this issue and
we commit to having this to you as quickly as possible.
